Dosage Exam Retake

NOTE: This is an individual assignment.  Students are expected to complete this quiz without the assistance of others, or the use of any outside resources.

All responses should be entered numerically (ex: 3) instead of spelled out (ex: three).

To pass the exam a score of 90% or higher is required. The dosage exam is 20 points, the score achieved on the first attempt will be the score that is recorded in the grade book and will not change regardless of the score you get on further attempts. A passing score of 90% must be achieved in subsequent attempts for successful completion of the course.  In order to obtain a 90%, you are only allowed to miss 2 questions. The date and time for the first exam is on the class schedule and completion of the exam during this time is mandatory.  In the case of illness or emergency, you must notify faculty prior to the time of the examination. Students who miss the dosage calculation exam without prior approval will earn a zero (0) for the missed exam. Students must pass the dosage calculation exam with 90% or higher to be able to successfully complete B260 clinical rotations.
