Generally speaking use of headings is a really good idea. If I may suggest the following for a template when completing case study assignments in this course: INTRODUCTION: this section will include the analysis of the case presented to you (remember you’ve been given the diagnosis, you just have to figure out what’s likely going on at this point of contact and layout a reasonable plan—supported by evidence of course); next use the rubric for a guide to minor headings: Pharmacokinetics/Pharmacodynamics…. Dosing/Cost/Other Economic Considerations… Follow up/Evaluation Plan. This is just a suggestion based on how my brain works but hopefully you get the idea. Using a process similar to this helps ensure content coverage and aids in clear and concise presentation of thought.

Assignment Instructions

Case Study: A 50 year old male is experiencing episodes of low back pain. He states that he is fairly sedentary and has put on a lot of weight recently. He states the pain is confined to the small of his back and gluteal region. The pain is worse when he gets up and moves about and lessens with rest. The pain has persisted for over 3 weeks. He has no history of serious illnesses. He rates the pain a “8” on a scale of 0 to 10.  He states that he gets some relief with Tylenol and Motrin.

Assessment Data:

Physical examination

Vital signs: BP 146/84, HR 84, RR 18, T37.1  BMI 32

Gait is normal and patient has no difficulty moving the joints of his lower limb.

Back ROM limited secondary to pain; Hip and LE strength 5/5, reflexes 2+

His straight leg raising test was negative.

There was no significant discrepancy in leg length.

No lumbar point tenderness.

Last PSA normal; UA negativ

Diagnosis: Low Back Pain
